CareContinuum uses a multi-disciplinary team approach to deliver
a higher level of customer service. By teaming clinical
expertise with result-oriented systems, and focusing on
early intervention, we are able to exceed our customers'
expectations.
To start the process, CareContinuum begins with assigning
each patient a primary case manager responsible for
reviewing medical necessity and for coordinating the entire
plan of care. Emphasis is placed on utilization review,
quality assurance, and a focused care management approach to
identify admissions/diagnoses that are potential high cost
drivers. When service is required for a patient, the case
manager discusses it with the ordering physician. If
appropriate, this conference may also include the primary
physician, medical director, care provider representative,
payer, social worker or others.
The Case Manager is the focal point for all communications.
Our goal is to get everyone to perform as a single
multi-disciplinary team. By creating a single focal point,
information flows...there's less duplication of
process...care is provided in a conscientious and
cost-effective manner.
